Total Aging In Place is a managed long term care health plan created as a way to provide you the flexibility to meet your health care needs. The goal is for you to continue living safely in the community.  A Care Team will partner with you to establish services that will support and promote your wellness and independence.  Our trained and professional Care Team can provide (if eligible):

-Social Day Program
-Adult Day Health Program
-Care Management
-Medical Transportation
-Home Care Services
                                                                           
To enroll in the Total Aging In Place Program you must be:
• At least 55 years old
• Live in Erie County
• Nursing home eligible
• Need the long term care service of the Total Aging In Place Program for at least 120 days
• Able to live safely in the community with assistance of the Total Aging In Place Program
• Medicaid eligible or willing to pay privately
