Individualized day program for adults with medical needs

Weinberg Campus offers an Adult Day Care Health Program for adults with medical needs. It is a community based program that provides comprehensive health care services in a congregate day setting. Participant needs are assessed and met through an individualized plan of care that is developed and implemented by an interdisciplinary team of medical professionals, including the participant’s personal community physician. We work with participants to maintain their wellness, allowing them to remain independent in their own homes, building their feelings of self-worth and increasing their self-confidence. Our facility is fully wheelchair accessible.

Weinberg Campus also offers a Garden Club program for the memory-impaired and a Social Day Care for respite support.
